How does Counterspell work in 5e?
As a reaction when you see someone within 60 feet casting: spells of 3rd level or lower fail automatically. Against 4th level or higher you make a spellcasting ability check against DC 10 + the spell’s level.
Do you need to know what spell is being cast?
By the rules you do not — you react to seeing someone cast. Many tables let you spend your reaction on Identify-by-sight first, but rules-as-written you are countering blind.

Yes, and it happens constantly at high tables. Counterspell is 3rd level, so another Counterspell stops it automatically unless it was upcast.
